What to Expect at Your First Chiropractic Appointment
We ask that you arrive 10 minutes prior to your appointment in order to fill out all required paperwork. You will be asked to fill out a patient registration form. You can complete this in advanced by printing the PDF document below. This will save time while in the office and improve accuracy to help both you and your doctor. Please bring the insurance card that you would like kept on file to your appointment.
When it is time for your appointment, Dr. Marty or Dr. Sarah will come out to greet you.
Expect to answer questions like the following:
"When and how did your pain start?"
"Where is your pain felt?"
"What activities/circumstances make your pain better or worse?"
